We provide consultation for patients suffering from common ailments to intractable diseases for a wide range of patients, from infants to elderly persons.

The Comprehensive Consultation Department provides integral consultation, including internal medicine and/or surgery. We provide consultation on common diseases, such as the common cold and backaches, as well as intractable diseases.


For persons with multiple symptoms, such as a history of cardiac arrhythmia and recently becoming forgetful, and for persons who are not sure which department to visit, please come to the Comprehensive Consultation Department.* If specialized medical care is required, we will introduce you to the appropriate facility.

We also offer specialized outpatient examinations, such as health checks and a smoking cessation program

The dermatology and orthopedics departments of the clinic are available to deal with child patients full time as a rule. However, a certified pediatrician is available to patients for consultation on Monday and Friday mornings in order to provide highly specialized pediatric healthcare.

Children of junior high school age or younger are requested to come with their parents.

This clinic believes a parent should be aware of the condition and illness of his/her child. Child visitors of junior high school age or younger should be accompanied by their parents.

In cases where a child visits unattended, his/her parent will be contacted by telephone before and after consultation. If contact cannot be established, consultation may not be possible depending on the decision of the doctor, etc.

To ensure safety for children too, the clinic is barrier-free and located on the 1st floor. Inside the clinic is barrier-free, and strollers can be used throughout the facility. The examination room and restroom are spacious.

An internal medicine specialist is available on Wednesdays (morning and afternoon) and Friday afternoon.

The internal medicine specialist provides early stage responses mainly for respiratory problems. Additionally, the doctor handles a wide range of diseases such as cardiovascular diseases, gastroenterological diseases and lifestyle-related diseases. For persons concerned about hair loss / thinning hair


The clinic can perform oral treatment for Androgenetic Alopecia (AGA) or male pattern baldness. This condition is commonly seen among adult men with thinning hair. Hair on the forehead and the top of the head starts to recede from either one or both sides after puberty. Generally heredity and male hormones are considered to be the main causes. As hair loss continues, thinning hair become more noticeable.

For persons concerned about “little,” “short,” or “thin” eyelashes.

"Eyelash hypotrichosis" is a deficiency or insufficiency in the amount of eyelashes.

Damage due to eyelash care (mascara, eyelash curling, false eyelashes and extensions, etc.), aging (length, thickness and density of eyelashes decrease with age.), skin disorders (loss of eyelashes due to skin diseases such as atopic dermatitis, psoriasis, etc.), and side effects of drugs (anticancer agents, etc.) are considered to be some of the causes.

What is osteoporosis?

A common belief is that as one ages, experiencing a fracture due to fragile bones is inevitable. In addition, a frequent reason for elderly persons becoming bedridden is falls and fractures.

However, recent medical progress has been significant, and determining the condition of fragile bones which may easily break due to metabolic disorders (such as osteoporosis) has become possible, enabling the prevention of bone deterioration as well as promoting their strengthening.

For people with a loss of height compared to when they were young or who have a recent history of fractures, it is possible to avoid becoming bedridden by starting treatment at an early stage.

Shinjuku Hiro Clinic provides osteoporosis outpatient care by Dr. Miyamoto. Dr. Miyamoto has considerable experience in osteoporosis treatment. There are many methods of treatment, and the most appropriate depends on the fracture history of the patient, severity, gender, as well as different drug administration methods.

Coordination with our expert staff

Avoiding falls, and strengthening bones are important in the prevention of fractures. Our osteoporosis outpatient department provides a program which is most appropriate for the patient and also provides advice on how to prevent falls, in coordination with physiotherapists who provide rehabilitation at this clinic or at patients’ homes.

Treatment which best suits the patients is selected after a comprehensive examination. At Shinjuku Hiro Clinic, a bone density test using DXA, the most standard bone quantity measuring device, is performed.

Specifically, we determine whether bone density is decreasing or not, by measuring the density of thighbones and the lumbar vertebrae, which are considered to become the most fragile due to osteoporosis. Calcium and hormone levels are measured by a blood test. In addition, bone metabolism markers are also measured.

We use the latest bone density measurement devices.

X-ray bone density measurement devices (bone densitometers) are used for femurs and lumbar vertebrae, and have the advantage of high accuracy. They can also provide accurate measurement in a short time.

Treatment agents for osteoporosis

In general, treatment is carried out using the following medicine to decrease the risk of fractures due to osteoporosis by 30- 50 percent,

  • Calcium
    Calcium inhibits the weakening of bones by providing calcium as a raw material of bones.
  • Activated vitamin D3
    D3 improves calcium absorption from intestines and accelerates bone growth.
  • Vitamin K
    Vitamin K activates osteoblasts and promotes bone growth.
  • Bisphosphonate agents
    Bisphosphonate agents inhibit bone resorption by osteoclasts. Bisphosphonate agents come in various types, such as oral medicine or injection, both of which are available.
  • SERM
    SERM is used to treat osteoporosis caused by decreased estrogen levels after menopause.
  • Parathyroid hormone agents
    Parathyroid hormone agents promote bone growth, but must be administered by injection. Parathyroid hormone agents comes in two types: a daily self-injection by the patient or a weekly injection by a medical institution.
  • RANKL antagonists
    RANKL antagonists inhibit deterioration in bone density by suppressing bone resorption by osteoclasts, similar to bisphosphonate agents. Though semi-annual injections are sufficient, combined oral medicine is required.

What is botulinum therapy (Botox injections)?

In this treatment, a drug, botulinum toxin, is injected into muscle tissue to relieve spasticity. This clinic uses the drug manufactured by Glaxo SmithKline.

To relieve tension in the muscles of the limbs, occurring as an aftereffect of a cerebral infarction.

It can be expected that botulism therapy will make everyday activities such as changing clothes easier.

Botulinum therapy has been approved by the Ministry of Health, Labor and Welfare for the following diseases.

  • Spasmodic torticollis, abnormal posture due to muscle tension in the neck and shoulders.
  • Equines foot associated with lower limb spasticity due to cerebral palsy.
  • Tension in the limbs due to a stroke, etc.
  • Hyperhidrosis of armpit

What is spasticity?

Spasticity is a motion disorder (dysfunction) often occurring after a stroke, where muscles become tense, and movement of the limbs is difficult or the limbs move unconsciously. In cases of prolonged abnormal posture due to spasticity, the muscles stiffen and joint motion becomes restricted (contracture), and results in difficulties in daily life. Since spasticity may hinder rehabilitation, treatment for spasticity is sometimes required.

Symptoms of spasticity (examples)
  • Clenched fingers which are difficult to open.
  • Bent elbows.
  • Toes are bent toward the back of feet.

The active ingredient in botulinum therapy

The active ingredient is a natural protein.

In botulinum therapy, an agent containing botulinum toxin as its active ingredient is used. Botulinum toxin is a natural protein produced by Clostridium botulinum (the bacteria which causes food poisoning).

Effect of botulinum toxin

Botulinum toxin suppresses nerve function leading to muscle tension. Muscle tension can be relieved by injecting botulinum toxin into the muscles.

Is there any risk?

In botulinum toxin therapy, the contagion in botulinum does not pose a risk because the bacillus itself is not injected.

Schedule and treatment details

Usually, the effect of botulinum toxin therapy lasts three to four months, and the effect gradually decreases after that. If treatment is discontinued, spasticity returns to its original state. Consequently, botulinum toxin therapy should be received several times a year. However, treatment frequency and intervals varies depending on the person.
